Mosai
Mosai makes software for home health and hospice agencies that handles referral intake, physician orders, and documentation review, with predictive models for visit levels and hospice transitions.
Strong fit
- Home health and hospice agencies that handle a high volume of referrals and physician orders
- Agencies that want predictive analytics on visit levels, hospitalization risk, and hospice transitions
- Agencies on Axxess, HCHB, WellSky, MatrixCare, or AlayaCare that want tools beside the EHR
Weak fit
- Agencies shopping for a full EHR with scheduling and billing
- Very small agencies with few referrals a week
- Buyers who need a published price list
Bottom line
Mosai is worth using for home health and hospice agencies that want faster referral intake and order turnaround and clearer clinical decisions without replacing their EHR. It suits agencies on Axxess, HCHB, MatrixCare, WellSky, or AlayaCare. Agencies that need a full EHR should look at Axxess or KanTime.
Score breakdown
Weights: Outcomes 35% · Product 30% · Implementation 20% · Pricing clarity 15%.
Mosai was formed in 2025 when two companies merged. Forcura, of Jacksonville, Florida, built referral and physician order software. Medalogix, of Nashville, Tennessee, made predictive analytics for home health and hospice.
The merger closed in March 2025, and the company took the name Mosai that October. Berkshire Partners is its majority owner, and The Vistria Group is its largest minority holder.
Mosai works alongside each agency's EHR. Mosai Referrals uses AI to pull data out of incoming referral packets and ranks them, so intake staff can accept or decline quickly.
Review checks documentation and coding. Pulse scores each patient's hospitalization risk and recommends a visit level. Transitions and Muse flag patients who may be nearing the end of life, and Orders tracks physician orders until they are signed and returned.
Mosai connects to Axxess, HCHB, MatrixCare, WellSky, and AlayaCare.
More than 1,000 home-based care providers use Mosai. In February 2026 Interim HealthCare named it the preferred workflow technology partner for its franchise network.
Interim HealthCare of the Great Lakes cut its order turnaround time 37%, from 6.48 days to 4.1 days. Munson Home Health brought its average referral processing time down to 30 minutes.
Agencies using Transitions have seen an average 52% drop in deaths on their home health census and 30% fewer short hospice stays. That means more patients reach hospice in time to benefit from it.
Compare Axxess or KanTime when the agency needs a full home health and hospice EHR. AlayaCare fits when home and community care need one online suite for scheduling, clinical notes, and billing.
HHAeXchange fits when the main need is confirming Medicaid home care visits and connecting to payers.
